How to File a Complaint:

- Complete the Complaint Form: Use the official form available on this website for timely processing. For any questions related to the complaint submission process or for any assistance needed, please contact a member of the Professional Conduct Department at complaints@clpns.com or call 306-500-1979.

- Provide Supporting Documentation: Submit evidence and a clear explanation of your concerns.

- File on Someone’s Behalf: Notify us if you are submitting a complaint on behalf of someone else and disclose your relationship to the individual.
Acknowledgment and Next Steps:

- CLPNS will acknowledge receipt of your complaint within two business days.

- CLPNS will share a copy of the complaint with the LPN, who will have the opportunity to respond.
